All-on-6 Dental Implants
All-on-6 distributes a fixed full-arch bridge across six implants, shortening cantilevers and adding redundancy — where bone volume makes the additional implants possible. Additional implants can reduce cantilever length and add redundancy, but they require sufficient bone volume and do not by themselves guarantee a better outcome.
Clear definition
What this treatment means
All-on-6 places six implants across the arch to support a fixed bridge. The additional posterior anchorage generally reduces or removes distal cantilevers and spreads occlusal load across more supports.
It requires more bone than a four-implant arrangement, particularly posteriorly, and may involve sinus augmentation in the upper jaw. Whether that trade-off is worthwhile is a planning decision, not a preference.
Choosing between four and six implants is made from CBCT anatomy, arch form, opposing dentition and bite force — not from a price list.
Problems addressed
- A fully edentulous arch with adequate posterior bone
- High bite forces or an opposing fixed arch
- A desire for greater redundancy in the support system
- Long arches where a four-implant cantilever would be excessive
Who may be suitable
- Bone volume permitting six implants, with or without planned augmentation
- Health and healing capacity for a longer surgical procedure
- Commitment to daily cleaning of a larger prosthesis
- Understanding of the additional cost relative to four implants
Limitations
- Requires more bone; augmentation adds time, cost and risk
- More implants means more surfaces to keep clean
- Additional implants do not compensate for an inadequate prosthetic design
- Not automatically indicated simply because it uses more implants
Alternatives
- All-on-4 where posterior bone is limited
- Segmented fixed bridges
- Implant overdenture
- Conventional complete denture
Assessment first
How the diagnosis is established
Records come before recommendations. The required investigations vary with the clinical question.
- CBCT assessment of posterior bone height and sinus anatomy
- Occlusal analysis, including the opposing arch
- Assessment of parafunction and expected loading
- Prosthetic set-up to determine tooth position and available space
- Hygiene access planning
Treatment journey
A sequenced clinical process
- 01
Planning
CBCT and prosthetic design confirm that six implant sites are available and useful.
- 02
Preparatory surgery
Extractions, ridge contouring and augmentation where required.
- 03
Implant placement
Six implants placed with distribution planned to minimise cantilevers.
- 04
Provisional prosthesis
Fixed provisional in suitable cases; otherwise an interim removable prosthesis.
- 05
Integration
Protected function while the implants integrate.
- 06
Definitive prosthesis
Passive-fit verification, framework trial and delivery.
- 07
Maintenance
Structured recall with prosthesis removal and component checks.
Prosthetic planning
- Load distributed across six supports to reduce stress concentration
- Framework designed for passive fit across a longer span
- Screw access channels positioned to preserve occlusal contacts
- Embrasure form designed for interdental cleaning aids
Materials considered
- Titanium implants with multi-unit abutments
- Monolithic zirconia or titanium-framework hybrid bridges
- Reinforced acrylic provisional prostheses
Informed decisions
Benefits, risks and maintenance
Possible benefits
- Reduced or eliminated distal cantilever in many cases
- Redundancy if one implant is later lost
- Load spread across more supports in high-force patients
- Option to segment the definitive prosthesis for easier repair
Risks and complications
- Longer surgery and, where augmentation is needed, additional healing
- Sinus complications where augmentation is performed in the upper jaw
- Peri-implant disease around a greater number of implants
- Prosthetic complications including screw loosening and material fracture
Typical duration
Typically four to nine months per arch from surgery to definitive prosthesis, longer where sinus augmentation is staged.
Recovery and adaptation
- Post-surgical swelling for several days to a week
- Extended soft diet where a fixed provisional is fitted
- Specific precautions after sinus augmentation
Long-term maintenance
- Daily interdental cleaning and irrigation beneath the bridge
- Professional cleaning with prosthesis removal at defined intervals
- Torque verification and component replacement over time
- Radiographic bone-level monitoring
What affects cost
- Six implants and associated abutments
- Any augmentation required to place them
- Provisional and definitive prosthesis materials
- Segmented versus single-span framework
- Number of arches treated
